Mobile Sitemap Issue

Hi there,
I am having some difficulty with an error on Webmaster Tools. I'm concerned with a possible duplicate content penalty following the launch of my mobile site.
I have attempted to update my sitemap to inform Google that a different mobile page exists in addition to the desktop page.
I have followed Google's guidelines as outlined here:
http://support.google.com/webmasters/bin/answer.py?hl=en&answer=34648I'm having problems with my sitemap.xml file. Webmaster tools is reporting that it is not able to read the file and when I validate it I am getting an error stating that the 'Namespace prefix xhtml on link is not defined'.
All I am trying to do is to create a sitemap that uses the rel="alternate" to inform Google that their is a mobile version of that specific page in addition to the desktop version.
An instance of the code I am using is below:

Any help would be much appreciated.

Make sure you're using the rel="canonical" tag on that mobile page. That tells Google that the original content is elsewhere on your site. This will help keep you from getting dinged from search engines.

A client implemented hreflang tags in the site header. MOZ says you aren't supposed to do an hreflang Sitemap as well. My question is how should I do a Sitemap now (or should I do one at all)?

Google Processing but Not Indexing XML Sitemap
Like it says above, Google is processing but not indexing our latest XML sitemap. I noticed this Monday afternoon - Indexed status was still Pending - and didn't think anything of it. But when it still said Pending on Tuesday, it seemed strange. I deleted and resubmitted our XML sitemap on Tuesday. It now shows that it was processed on Tuesday, but the Indexed status is still Pending. I've never seen this much of a lag, hence the concern. Our site IS indexed in Google - it shows up with a site:xxxx.com search with the same number of pages as it always has. The only thing I can see that triggered this is Sunday the site failed verification via Google, but we quickly fixed that and re-verified via WMT Monday morning. Anyone know what's going on?

Canonical url issue
Canonical url issue My site https://ladydecosmetic.com on seomoz crawl showing duplicate page title, duplicate page content errors. I have downloaded the error reports csv and checked. From the report, The below url contains duplicate page content.

Should I create a separate sitemap.xml for paginated categories?

Sitemaps: Alternate hreflang
Hi, some time ago I have read that there is a limit of 50.000 URLs per sitemap file (So, you need to create a sitemap index and separate files with 50.000 urls each). [Source]. Now we are about to implement the link hreflang in the sitemap [Source], and we dont know if we have to count each alternate as a different url. We have 21 different well positioned domains (Same name, different cctlds, a little different content [varies in currencies, taxes, some labels, etc] depending in the target country) so the amount of links per url would be high. A) Shall we count each link alternate as a separate url, or just the original ones? For example, if we have to count the link alternates, that would make us have 2380pages per sitemap, each with one original url and 20 alternate links. (Always being aware of the 50mb maximum filesize) B) Actually we have one sitemap per domain. Using this, shall we generate one per domain using the matching domain as original url? Or it would be the same if we upload to every domain the same sitemap? Thanks

Technical SEO issue
Hi Everyone, I have encountered a major issue in one of my clients website(kitchen appliance website). This client has 2 main websites (A & B) linked with each other representing 2 different categories of appliances. We are trying to create some brand pages that this store carries. One brand page has been created and when searching for it on SERP, the results found should be under URL A but it is under URL B. I don't know what is going on? Can someone explain me what happened? Can a XML sitemap index point to other sitemaps indexes?
We have a massive site that is having some issue being fully crawled due to some of our site architecture and linking. Is it possible to have a XML sitemap index point to other sitemap indexes rather than standalone XML sitemaps? Has anyone done this successfully? Based upon the description here: http://sitemaps.org/protocol.php#index it seems like it should be possible. Thanks in advance for your help!
