Brand Appearing in the Meta tilte by default

Hi All,
Has anyone experienced the home page meta title indexed with their Brand coming up first before their actual Meta title? I know google overrides the meta title sometimes when the title is not relevant, I understand that. This just started happening 6 months ago. I thought I would leave it to see if it get corrected. but no luck! Any suggestions? Many thanks

2 version of meta description and title
Hi guys, I have seen two versions of meta descriptions and meta titles for one URL (this is happening on a few of my pages). Initially, I thought Google is pulling the information from my page content. However, it is not the case. I have realized the information showing on SERPs are very old description (years ago information). Just a bit background information, we had a new website launched in April this year. The issue stated since the new website launch. Anyone experienced similar situation?

How many times should a word appear in an article?
Hi My site is maily about growing fruit and vegetables and was badly hit after the September Panda update. Each page is about one type of fruit or vegetable and discusses how to grow that particular plant. Each page targets two keyword phrases, "growing abc" and "how to grow abc" so e.g. "Growing garlic: a guide on how to grow garlic" would be the page title and then the two phrases used a couple of times on each page. However the plant, in the above example "garlic" can appear upto 30 or 40 times per page, also the phrases "how to grow" and "growing" will appear on just about every page, would these facts be hurting my overall site rankings? Meta Keywords, Should I just remove them?
howdy guys, I am helping my father out with his SEO for his company. I am taking over the SEO helm so to speak and have to do a complete on site make over( offsite as well eventually soon) He has on every page meta keywords that are just stuffed with "money keywords" and they are the same on every page, Should i just delete them entirely and leave that attribute blank? What does the "base href" meta tag do? For SEO and webdesign?
I have encounter the "base href" on one of my sites. The tag is on every page and always points to the home URL.

Keyword use in meta description
Anyone know if its important that the target keyword is used AT THE START of the meta description, or is it enough that it is anywhere in the description...? I have a few big ones that are at the end of the description and I was wondering if I should move them up to the front... Thanks!

Meta descriptions
Whats the deal with the date at the start of the meta descriptions? I have not really looked into this but I'm guessing its a blog thing?
