Confounding "Accessible to Engines" error?

Most of the pages on our site "Accessible to Engines" test in the SEOmoz reports. We cannot find any problem with the code and it's largely identical to the few pages that come up with an "A" score. One item that may be a reason is that we use
meta http-equiv="refresh" content="600;
For example in www.weatherzone.com.au/nsw/sydney/sydney
We use this to fresh dynamic content on our site. Do search engines penalise pages that use this form of page refresh?
Alternatively, is there a known bug in the SEOmoz "Accessible to Engines" report?

Thanks for responses. I received Aaron's reply.
However, he did not answer the critical question whether the use of the "meta refresh" tag penalises SEO performance as he indicated that he is not an SEO expert. Aaron suggested I post the question here to obtain a more authoritative answer.

I see some links on Youtube.com that look like they are "Do-follow" links am I wrong?
Hi, I have some videos on YouTube.com and I notice many of the links show as "Do-Follow" per the Moz Bar. I have a feeling they are not actually "Dofollow" links because they never show up in Moz Pro, SemRush or any of my other tools. Can some of you people with greater knowledge of YouTube please clarify why the links look like "Do Follow". Why is moz saying I have a 404 error?
I recently asked a question on here about how to fix my 404 error. I figured it out, and now webmaster tools says it's not there any more. This as been confirmed by keeping watch for about a week now. But Moz just crawled my site today and is for some reason showing an error. Am i missing something here or is Moz a little slow sometimes?

Question about Crawl Diagnostics - 4xx (Client Error) report
Hi here, I was wondering if there is a way to find out the originating page where a broken link is found from the 4xx (Client Error) report. I can't find a way to know that, and without that information is very difficult for me to fix any possible 404 related issues on my website. Any thoughts are very welcome! Thank you in advance.

"Loading" error in Open Site Explorer

Duplicate page error from SEOmoz
SEOmoz's Crawl Diagnostics is complaining about a duplicate page error. I'm trying to use a rel=canonical but maybe I'm not doing it right.
