How do I fix the error duplicate page content and duplicate page title?

On my site www.millsheating.co.uk I have the error message as per the question title.
The conflict is coming from these two pages which are effectively the same page:
I have added a htaccess file to the root folder as I thought (hoped) it would fix the problem but I doesn't appear to have done so.
this is the content of the htaccess file:

Hi Jason,
A much more simple fix would be to add a to the /index page. Google will attribute any page authority back to the root and not count the duplicated page.

Looking for opinions on structuring meta title tags/page title/menu title/H1
Hi everyone I am hoping a few of you can share your opinions. I have been having conversations (okay, healthy debates) about how to write/structure meta title tag and how to compliment them with the H1, page title, menu name. To help explain the thought processes I will use a pretend keyword. How about "screwdriver". Case: (I made this up) we are redesigning a website for a construction tools manufacturing company (pretend name: ABC Tools) targeting OEMs who are interested in purchasing large quantities of tools. The product categories (to become main menu items) are Screwdrivers, Nails, Drills, and Hammers. (bear with me .... this is just an example I am making up on the fly) K. Circling back to screwdrivers - let's say we have one landing page (a primary category page and in the main menu) listing products and great details about screwdrivers. Focus keywords are screwdriver manufacturer, screwdriver supplier, construction screwdrivers Below are questions being debated. If you are willing ... how would you address these questions? And, can you explain WHY? QUESTION ONE: How would you structure the meta title tag (feel free to write one of your own) Screwdriver Manufacturer - Construction Screwdriver | ABC Tools ABC Tools - US-based Screwdriver Manufacturer Supplier Near You High-Quality Screwdrivers for Construction with ABC Tools QUESTION TWO: how would you write the H1 on the page? Would it match the meta tag? OR, would you write something different using the primary keyword? QUESTION THREE Remembering this is not a blog post ... it is a primary landing page linked to the main navigation. What would the menu title be? (remember the product categories above are how the main menu items are bucketed) Screwdrivers Screwdriver Manufacturer Typically in WordPress, the H1 and the menu title is auto-populated using the page title (not the title tag)... So, if we use Screwdrivers as the page title but we want the H1 to match the meta title tag, would we manually change the H1? Or, have the page title and title tag match, but manually change the menu item?

Is This Considered Duplicate Content?
My site has entered SEO hell and I am not sure how to fix it. Up until 18 months ago I had tremendous success on Google and Bing and now my website appears below my Facebook page for the term "Direct Mail Raleigh." What makes it even more frustrating is my competitors have done no SEO and they are dominating this keyword. I thought that the issue was due to harmful inbound links and two months ago I disavowed ones that were clearly spam. Somehow my site has actually gone down! I have a blog that I have updated infrequently and I do not know if it I am getting punished for duplicate content. On Google Webmaster Tools it says I have 279 crawled and indexed pages. Yesterday when I ran the MOZ crawl check I was amazed to find 1150 different webpages on my site. Despite the fact that it does not appear on the webmaster tools I have three different webpages due to the format that the Wordpress blog was created: "http://www.marketplace-solutions.com/report/part2leadershi/", "http://www.marketplace-solutions.com/report/page/91/" and "http://www.marketplace-solutions.com/report/category/competent-leadership/page/3/" What does not make sense to me is why Google only indexed 279 webpages AND why MOZ did not identify these three webpages as duplicate content with the Crawl Test Tool. Does anyone have any ideas? Would it be as easy as creating a massive robot.txt file and just putting 2 of the 3 URLs in that file? Thank you for your help.

Pages Returning A 403 Error
Hiya Moz Community I hope you are all great, I have a question regarding one of my websites, I have the main site and 2 sub folder sites essentially, I decided to upgrade one of the sites and placed it in a different sub folder, I then set up a 301 redirect to the new location, so far so good, I have been having a look at my link profile using AHrefs, inside there is an SEO report facility, I ran the report and I have over 500 pages returning a 403 or Forbidden error. my question is whether the Equity from those pages is being passed to the new site? I actually removed all the old site from Google Cache to avoid misleading visitors, I suppose I could set the re-directs up manually if I the equity is not being passed to the new site although I was under the impression it would be, or 85% - 90% of it would be anyway. The reason why I am asking is that I have seen a significant drop in rankings for keywords that my site has always ranked highly for. thought I would see if you guys can clear that up for me. Is Sitemap Issue Causing Duplicate Content & Unindexed Pages on Google?
On July 10th my site was migrated from Drupal to Google. The site contains approximately 400 pages. 301 permanent redirects were used. The site contains maybe 50 pages of new content. Many of the new pages have not been indexed and many pages show as duplicate content. Is it possible that there is a site map issue that is causing this problem? My developer believes the map is formatted correctly, but I am not convinced. Duplicate content even with 301 redirects
I know this isn't a developer forum but I figure someone will know the answer to this. My site is http://www.stadriemblems.com and I have a 301 redirect in my .htaccess file to redirect all non-www to www and it works great. But SEOmoz seems to think this doesn't apply to my blog, which is located at http://www.stadriemblems.com/blog It doesn't seem to make sense that I'd need to place code in every .htaccess file of every sub-folder. If I do, what code can I use? The weirdest part about this is that the redirecting works just fine; it's just SEOmoz's crawler that doesn't seem to be with the program here. Does this happen to you?
