Why can I see 404 pages in Google Analytics but nothing in the On-Demand Crawl?

Hello,
I'm looking at some Google Analytics data for a website and can see a few 'Page not found's among the Page Titles, looking like these are 404 errors.
To get a full list of what's 404-ing so I can get these redirected, the Moz on-demand crawl of the website has come back with no major errors and just a few metadata ones.
Does anyone know any potential reasons why the audit has drawn a blank, and is there another way to get a comprehensive list of 404s, as I'm aware the Google Analytics data may not be covering all of them.

example : https://www.csgosmurfkart.com Site's robots.txt settings are good. and I think there's no blocking factor. But On Page Grader cannot crawl the sites.
But campaign crawler is working well on the site. only On Page Grader is not working.. What should I change my server's setting or site's setting for crawling site on my site?

Site crawl warning - concatenated urls from Wordpress
I could use some help on how to fix this. I asked at the walkthrough but was told it was a Wordpress issue but so far I can't find anything to point me in the right direction. There are no errors in the files on server side and I have asked my hosting company too. I am hoping someone here may be able to shed some light on it. One of my websites it giving 404 errors on links that are formed as below and there are over 12.7K of them! Example: <mydomainurl>/www.instagram.com/www.instagram.com/<instagram username=""></instagram></mydomainurl> The link that relates to my website is valid and working, but I don't understand the rest. I am totally stumped on how to move forward with this. Any advice, suggestions, tips on how to fix these errors and stop these types of links getting generated. Thanks.

Re On-Page Grader
One of the pages I'm trying to optimise is achieving an 'A' grade, however all the ticks are black not green as I've seen on other page grade. Why is this? Help much appreciated. Thanks

Has using Moz got me banned from Google search?
Dear fellow Mozzers, For the last couple of weeks when ever I do a google search I have to enter a captcha code. Now I hardly use google anymore which is annoying ! Is there a way to see keyword density by percentage?
My management wants us to track the percentage of keyword density. While I don't feel this is as important as it once was, they want it nevertheless. I don't see a way to get that information. I thought it would be in the On-Page Grade, but if it is there I'm missing any. Is this possible in Moz Pro? Thanks,

Suggestion for Improving the Crawl Report on Canonicals
This came up in the answer to a question I gave here http://moz.com/community/q/canonicals-in-crawling-reports#reply_222623 Wanted to post here to put it in as a suggestion on how to improve the Moz Crawl reports Currently, the report shows FALSE if there is no canonical link on a page and TRUE if there is. IF you get a TRUE response, this shows up as a warning in your report. I currently use Canonical to Self on almost all my pages to help with some indexing issues. I currently use the EXACT function in excel to create a formula to see if my canonical link matches the URL of the page (as this is what I want it to do). I can then know that the canonical is implemented properly, or if I need to manually check pages to make sure the canonical that points to another page is correct. I would like to suggest that the Moz crawl tool does this. It can show FALSE is the canonical is missing, TRUE if the canonical is present and SELF if the canonical points to the URL of the page it is on. I think for the most part this would be much more actionable information. I would even suggest that TRUE would need to be more of a high priority alert, and SELF can't do any damage, so I would leave that info in the CSV but not have that as a warning in the web interface. Is Moz Analytics still available to all MozCon attendees?
I thought all Mozcon attendees were going to get access to Moz Analytics even after the convention was over. I can't seem to find were to access it from my campaigns manager dashboard and the Analytics landing page says im on queue to get access.
