Moz Local not recognizing my CSV Listing File After Multiple Attempts

I'm trying to get Moz local to recognize my csv file, and its not seeming to do so. I've tried using Google Spreadsheets, OpenOffice Calc, and have even used a Moz Local generator in Google Docs. I've tried this a dozen times now, and none of them seem to work, and leave me with the error --
We did not recognize the file type you uploaded.
Please make sure the file you upload is in CSV (Comma Delimited) format. You may need to re-save or export the spreadsheet as a CSV. Please note that the column headers are required, and must not be changed.

Local Moz - documentation for Acxiom
Hello there, Moz Local shows that I am not listed with Acxiom, so I went directly to the Acxiom website to add my business listing. During that process, Acxiom asks for business documentation to be uploaded (DBA, license, Federal ID, etc.). If I were using Local Moz to send the data out to the data aggregators, how does it handle this portion (uploading business license, etc. as required)? I'm hoping to use Moz Local for my website clients but need to understand this bit first. Thanks for your help!

How long does it take for the MOZ Local to update?
Hello, I use MOZ local to keep track of major citations for 30+ clients. Sometimes, it happens that I would be working on a new company, which does not have any significant number of active citations. And MOZ local does not seem to update very often, when I have already created citations for that particular new company. Can anyone update me about, what is the normal timeline for the MOZ Local to update new citations? I am using this URL to keep track of my company citation ranks: moz.com/local/search. Why are there 2 Managed items when I've only got one Local Campaign running?
In my Local Dashboard, there is a green circle with the number 2 in same.... Yet I've only got 1 Local campaign ongoing.... ???
